The upper facial contour includes the forehead, temples, nose, eye orbits, upper jaw (maxilla), malar bones (cheekbones), and temporal processes.

  • Forehead
    It can lose central projection as a result of the decrease in fat tissue.
  • Temples
    Hollowing or loss of volume in the temples modifies the contour of the orbits and alters the balance with the lower facial contour.
  • Upper jaw (Maxilla)
    The loss of fat tissue in the infraorbital region, on both sides of the nose, can create an appearance similar to dark circles.
  • Malar bones (Cheekbones)
    In youth, the cheekbones have a fat covering that softens their contours. Over time, this coverage decreases, leaving structures more exposed and a more angled appearance.
  • Temporal processes
    They correspond to the region closest to the scalp. By losing the fat covering, the temporal process bone becomes more evident.

The lower facial contour comprises the lower jaw (mandible), chin, perioral region, nasolabial folds, and mandibular contour.

  • Lower jaw (Mandible)
    The mandibular line loses continuity when the fat pads of the cheek redistribute and descend, accumulating over the mandibular border and giving rise to jowls.
    Although skin tightening through a rhytidectomy (facelift) may be tempting, in some cases it fails to completely restore the continuity of the mandibular line. In these situations, it may be more effective to disguise this irregularity through a slight lengthening of the cheek, allowing a visually straight and continuous mandibular line to be maintained.
  • Chin
    The most characteristic alteration of the chin is the loss of lateral volume, with a predominant downward projection. This change makes the contour narrower and acquire a more point-like shape, oriented inferiorly, which modifies the balance of the lower third of the face.
  • Perioral region: In the perioral region, a deflationary phenomenon occurs, characterized by the overall thinning of the entire perioral unit. On the periphery of the perioral area, nasolabial folds and marionette lines are accentuated. In the area close to the lips, vertical grooves begin to be observed, described by patients as “piano keys” (smoker’s lines), along with a progressive thinning of the vermilions. In some cases, horizontal grooves also appear on both vermilions. The correction of these changes is not done using volumizing lip profiling fillers, which have a greater consistency and can alter the natural shape. Instead, different types of hyaluronic acid are combined: in true hollows, fillers are used to level the defect, and throughout the perioral region, formulations aimed at tissue revitalization are used, which improve tissue quality without generating an excessive volume increase. This approach allows for a natural recovery of functional volume and the correction of hollows, as an expression of the overall loss of fat tissue in the area.
