The current state of the art of laser acne management has the following indications:

  1. Drainage: Presence of inflamed comedones, pustules and nodules. Laser is used to facilitate drainage (Erbium-YAG with fine tip) and to reduce inflammation (Blue light, IPL, Nd-YAG, Fraxel 1550)
  2. Scars: Recent or old red scars.
  3. Contraindications for oral isotretinoin: Patients in whom this medication is contraindicated and control is not achieved with hormonal therapy or antibiotics.

Contraindications and precautions:

  1. High-dose oral isotretinoin: Makes it impossible to use fractional ablative lasers
  2. Use it as a sole therapy in cases of persistent appearance of new lesions

9-year history of a case with persistent active acne for years in the anterior neck region who initially had comedones drained but over time had to have a large number of skin bridges removed in the 5th year of treatment. Microsurgical procedures were supplemented with Fraxel 1550 laser. Improvement has persisted since then.
